fairing. Decals from other OIL companies MAY NOT
be displayed!

Racer MUST list SILKOLENE
as a sponsor.

Racer MUST fill out and sign
this form at EACH and EVERY race event.

Use one form per motorcycle per race
day.

Racer MUST turn this form into
Club Sign-up or Tech. Inspection BEFORE competition.
Contingency claim form
is completed in full with the rider's social security number, legible
address, and name submitted before each race.
